Advance care planning is a way to help you think about, talk about and share your thoughts and wishes about future health care. It gives you a "voice" in decision making, helps you determine who would communicate for you if you are unable to communicate for yourself and should include conversations with your health-care team.
Everyone should have a say in their health care. Plan ahead today to ensure that your wishes are known, no matter what the future holds for your family and your health.
A legacy is a gift from one generation to the next.
Legacy Projects provide a structured way to collect, save, and store memories with historical information and stories, photographs, letter or cards, articles and documents gained throughout a lifetime. Events, achievements and/or belongings can be creatively memorialized to leave for family and friends to share and become an heirloom for future generations. It is a constructive and therapeutic way of “commemorating” a piece of a person's history and making a tangible article representing the memories and precious events which represent an individuals life in a family's story.

Yours in Life and Death offers Elder, Respite and Vigiling services by the hour or in pre-planned packages. Packages are personalized to the service required. You can have the same Doula attend daily or arrange for continuous support as we work with and within a network of other End Of Life Doulas, professionals and volunteers to provide holistic care.
We can visit your elderly family member in their residence and/or provide respite for up to 6 hours. Depending on the circumstances, light housekeeping in the home environment can be available upon request. We will also visit in long term care facilities or hospitals to provide companionship and comfort. If your family member only speaks Portuguese, we can be a translator to bridge the language gap in a facility setting or with home care staff.
Together, we can plan the vigil for how you want your final days and hours to be carried out, giving you peace of mind knowing your wishes will be honored. Most of life's significant events require planning and our final journey into death is no different. While we can never be sure of the day or the hour, you can be sure your specific wishes will be followed as you transition into death. The best practice is to always be prepared. It's a difficult conversation, but a plan; whether at home or in a facility will safeguard your preferences. (Please note there will be much more flexibility in a home setting, but we will strive to personalize your wishes to the very best of our ability no matter where your plan takes place).
